Grant Questionnaire for Prospective Planters

Please complete this quick questionnaire below. It helps the TIMES12 Central team allocate grant resources thoughtfully and prayerfully in alignment with our mission and values. While we are committed to supporting gospel-centered church planting in many different contexts, we recognize that we cannot fund every opportunity at the same level. This tool allows us to assess both the strategic importance of the planting location and the planter’s demonstrated capacity, calling, and alignment with TIMES12.

This grant questionnaire should be completed by the Hub Director after the assessment has been completed. We need this questionnaire completed in order to complete the Compensation Agreement.

PROCESS:

Lead Assessor conducts the all-day assessment for the prospective planter and completes the Assessment Report.
Lead Assessor reviews the Assessment Report and the HHH grid with the Hub Director (if not the same person).
The Hub Director completes this form at least 48 HOURS IN ADVANCE of the orientation & onboarding meeting so the comp agreement can be completed and uploaded to the prospective planter’s drive folder.

• The under-resourced, poor, or marginalized. They may not be strategic for sending potential among cultural influencers, but we believe these subcultures are on God’s heart and we are commanded to love the poor.

• May not have the strategic reasons listed above, but lost people matter to God so we want to encourage planting wherever we can. Healthy plants can also contribute people and financial resources for future planting in a variety of locations.
